DIAL 911 AND CONTACT LAW ENFORCEMENT IMMEDIATELY
ON 11/27/17 AT APPROXIMATELY 8:00 A.M., JACK MARTIN WAS LAST SEEN DRIVING AWAY FROM HIS HOME WHICH IS LOCATED IN THE AREA OF 11TH AVENUE AND THUNDERBIRD ROAD, PHOENIX, ARIZONA. HE WAS DRIVING A SILVER, 1996, TOYOTA TACOMA PICKUP WITH ARIZONA LICENSE PLATE NUMBER 380WXY. HE WAS WEARING A RETIRED NAVY, BLUE HAT, PRESCRIPTION GLASSES AND A PLAID SHIRT. HE HAS BEEN DIAGNOSED WITH EARLY STAGES OF DEMENTIA. HIS FAMILY BELIEVES THAT HE IS LOST.
