On 05/21/19 at approximately 3:30 P.M., Charles S. Martinez was last seen in the area of 22nd Street and Sweetwater Avenue, Phoenix, Arizona. Charles has dementia and he might be lost.
Charles was last seen wearing a light blue baseball cap with an unknown logo on it, a dark blue jacket with Conn's embroidered in red on the upper front left side, a black sweatshirt, black pants and black shoes. Charles was carrying a green gym bag with clothes inside it.
